Disable focus grabbing on start, fix dock icon w/o window

Google QT_MAC_DISABLE_FOREGROUND_APPLICATION_TRANSFORM

ust setting that env var prevents focus stealing, but this includes for the actual windows.

Also, doesn't hide the app icon
