I have tried something like this before and the extension always fails. Where do you get these particular files? Is it 2.0.3 alpha?

I also note that the date modified on SSMSExecutor.pkgdef is more recent the data of 2.0.3 alpha. Did you make any changes?

It seems I have in installed in the same folder like you:
C:\Program Files (x86)\Microsoft SQL Server Management Studio 19\Extensions\SSMSExecutor
