On first run - got an error on opening an issue

Seems that the scanner found some a11y issues, but it fails, to my understanding, trying to create an issue.
I don't understand why it refers to the docs website.

The error I see:

Token problems are a common cause of “HttpError: Not Found” errors during issue creation. “2. Create a Token and Add a Secret” documents token usage; in summary:

1. Create a fine-grained PAT with the actions: write, contents: write, issues: write, pull-requests: write, and metadata: read permissions and access to both the repo containing the workflow and the repo where issues will be filed (or, alternatively, a classic PAT with the repo scope).
2. Add your PAT as a repository secret (e.g. named GH_TOKEN) in the repo containing the workflow.
3. Ensure your workflow file references the secret you added (e.g. token: ${{ secrets.GH_TOKEN }}).

If you get the same error message after reviewing these steps, then could you please link to (or embed) your workflow and workflow run logs? (If any sensitive data, e.g. tokens or passwords, is included, please remove it before sharing.)

I'm also getting this error. I've followed the fix suggested but the error persists. Here's the link to the workflow run logs. Any help?
