Summary

add configurable hotkeys for focused-app volume up, volume down, and mute toggle
add a shared foreground-app resolver and reuse it from the Actions addon
expose the new focused-app shortcuts in the EarTrumpet shortcuts settings UI

Notes

this follows the existing app-level hotkey flow instead of introducing a separate hook system
modifier combinations are supported through the existing hotkey infrastructure
