It takes a long time to index a new server, but we index the same cards repeatedly.
This PR uses the permissions and filesystem contents, the url and if there are published realms within a server and in one specific startup (setupPermissionedRealms) it will cache the database as a template database after initialising. Then when the same contents are requested, start by populating the database with the already indexed content.
Also some cleanup removing realm servers we never use, improving caching etc.
