docs: retarget m-tools attribution from archived org repo to rafael5/m-tools#46

The m-tools seed repo was moved out of the org back to its original
author's account (now at github.com/rafael5/m-tools), not archived
in-place as the earlier wording assumed. Update every reference so
links resolve to the live repo and the rehosting narrative reflects
what actually happened.
- profile/README.md: history pointer rewords "(now archived) m-tools"
→ "m-tools seed repo (since moved out of the org back to its
original author's account)".
- docs/history/{README.md,m-tooling-tier1.md,m-tool-gap-analysis.md,
gap-analysis-and-remediation-strategy.md}: archived-snapshot
banners + sibling-doc links updated to point at rafael5/m-tools.
docs/history/README.md gets a fuller rewrite of the provenance
narrative + a new "preserved here" section explaining what each
rehosted file is and why.
- docs/ai-discoverability/AI-discoverability-plan.md: m-tools row in
the per-repo table + §11 operational-invariant #4 reworded.
- docs/ai-discoverability/README.md: history subsection mentions the
rafael5/m-tools source.
- docs/ai-discoverability/phases/phase3-plan.md: tier-2 closure note
describes m-tools as "out-of-org" rather than "archived".
No catalog or schema changes; tools.json doesn't reference m-tools
(carryover entry was dropped pre-Phase-1; see project memory).
