Add metadata extraction (title, author, subject, keywords, creator, producer, creation/modification dates) from PDF document info dictionaries using pdfminer. Metadata is formatted as a bullet list and prepended to the converted markdown output with a horizontal rule separator. The title field is also set on DocumentConverterResult when present, enabling downstream consumers to access it programmatically. Metadata is only included when the PDF has extractable text content, so scanned/image-only PDFs remain unaffected. Fixes microsoft#1664

Purpose
Fixes #1664. Extracts PDF document metadata (title, author, subject, keywords, creator, producer, creation/modification dates) and prepends it to the converted Markdown output.
Problem
The
PdfConvertercurrently extracts only the text body of a PDF, discarding structured metadata from the document info dictionary. For research, legal, and LLM workflows, this metadata provides essential context for citation, provenance, and document management.Solution
